We are open M, T, TH 9-5 W, F 9-2. Hours subject to change without notice. Closed on holidays.
Payment and Financing:
To maintain our commitment to offering quality care, full payment is required at the time of service for all office visits, supplements, treatments, and lab work. We accept HSA payment. But it is the patient’s responsibility to know what their specific HSA plan will and won’t cover.
Why We are a Cash-Based Model:
Our cash-based approach enables us to offer you more time and attention without the restrictions set by insurance companies. This model allows us to provide comprehensive care tailored to your needs, directly aligning our work with your health goals, rather than insurance mandates. We believe we need more than 10-15 minutes with our patients, and AIM to provide a higher quality service for our patients. We work for you, not your insurance company.
Insurance and Reimbursements:
While we do not process insurance claims, we're happy to provide documentation to help you seek reimbursement from your provider. Please inform us if you require assistance with this process. This is processed for a nominal fee of $30.
Prescription Policy:
Prescriptions will only be issued following a consultation with one of our practitioners. In certain cases, blood labs may be required before any prescription is provided. This ensures that all medication is prescribed based on the most current and comprehensive understanding of your health needs. We recommend planning ahead for prescription refills to avoid any gaps in your medication regimen. Contact your pharmacy for any and all refills. Please allow 48-72 hours for processing.
Cancellation Policy:
We recognize that life is unpredictable and schedules can shift. To ensure we can provide the best care to all our patients, we kindly ask for a 48-hour notice if you need to cancel your appointment. For cancellations made after this window, a $50 fee will be charged. In the event of a no-show, you will be billed for 50% of the appointment's value. New patients should be aware that if you miss your appointment, your deposit will be non-refundable under this policy. For IV therapy appointments, cancellations made on the same day as the appointment will incur the full cost of the IV treatment.
We also understand that emergencies happen. In such instances, the decision to waive the cancellation fee will be at the discretion of your provider. To accommodate other unforeseen circumstances, we offer the flexibility of rescheduling your appointment as a phone or telehealth visit.
Emergency care:
Our clinic does not provide on-call after-hour services and nor do we provide emergency medical service or admit or care for patients in the hospital.
Email and Messaging:
There is no charge for clarification of your treatment plan directly following an appointment, medication dosing, or when your physician has requested that you check in about your response to a treatment. If you have a new medical concern that arises or it’s the appropriate time to follow up with your provider, please set up a follow-up appointment for discussion and treatment planning.
Documentation Policy:
Requests for doctor's notes, letters, or forms will necessitate a consultation with a practitioner. This policy allows us to provide accurate and personalized documentation that reflects your current health status and needs.
Health and Supplement Recommendations Policy:
No health/medical advice or supplement recommendations will be provided by our front desk staff or health providers. Any inquiries related to health concerns or conditions will be directed to schedule a visit with a practitioner, ensuring you receive expert, tailored advice.
Medical Records:
Your medical records are subject to HIPAA policies. We will need direct consent in order to release records regarding your care with us. We can provide a consent form to sign in order to have your records released to your primary care physician or any other person/entity.
Custom Lab Orders:
Custom lab orders will need to be addressed in an appointment. Lab results will also be discussed exclusively during an appointment with your provider, ensuring you receive a comprehensive review and understanding of your results.
Membership Model Considerations:
These policies have been thoughtfully incorporated into our membership plans. Members can rest assured that these services are included, reflecting our commitment to providing seamless, comprehensive care through a structured, supportive framework.
Supplement Purchases and Returns:
All supplements must be paid for at the time of purchase. Special orders must be paid in full before ordering. Unopened supplements can be returned for a full refund if returned within 30 days of original purchase. Refunds are not valid on injectables, injection supplies, or refrigerated items. Refunds are not offered on services already rendered. We can mail supplements and small items. Shipping fees will apply. We can hold items until the end of the day for pick up but will be put back on the shelf if the item is not paid for.
